Thank you for your interest in Treehouse Pre-School! This page contains everything you need to enrol your child with us. There are two types of information:

 

1. In the green box you will see a link to our enrolment policies and procedures. These require no further action from you apart from a few minutes of your time to read through them. 
    

2. You will need to fill in a few forms. Please ensure you provide the most accurate and up-to-date information you can so our staff can provide your child/children with the most appropriate care and development opportunities.  

 

The forms we require you to complete and return are:

 

• Child Information form

• Conditions of Provision (please sign and return this form and print a copy for your records)

• Booking and fees policy 

• Session Booking form

 

The links for all these forms can be found in the green box. If you are applying for a funded place, you will also need to complete a Parent Declaration Form from Buckinghamshire County Council. At present these are only available in hard copy format. On receipt of the above forms, we will post one out to you. Please note that if you do not complete this form, sessions booked will be payable. We will need to see original documentation to prove your child’s date of birth e.g. birth certificate, red book, alongside this completed form. If you decide to deliver the forms back to us by hand, please bring this with you then, otherwise, please remember to bring it with you to the first session that your child attends.
